【摘要】：當(dāng)前我國醫(yī)藥流通市場進(jìn)入戰(zhàn)略轉(zhuǎn)型期,市場增長趨緩,行業(yè)競爭加劇,將醫(yī)藥供應(yīng)鏈服務(wù)延伸至醫(yī)院降低運(yùn)營成本擴(kuò)大市場占有,是醫(yī)藥流通企業(yè)競爭的新領(lǐng)域。同時隨著醫(yī)改深入藥品加成率取消和新版GSP實施帶來的監(jiān)管加強(qiáng),醫(yī)院藥學(xué)部門從原有盈利部門轉(zhuǎn)為經(jīng)營負(fù)擔(dān)。降低用藥成本,提高醫(yī)院運(yùn)營效率成為了醫(yī)院經(jīng)營者的關(guān)注重點(diǎn)。SPD模式是近年引入國內(nèi)一種醫(yī)院內(nèi)藥品供應(yīng)鏈管理模式,其將第三方物流管理理念和集中化管理思想融合,是一種有效降低藥品流通成本和改善醫(yī)院用藥管理水平的方法。本課題從供應(yīng)鏈管理的基本概念入手,討論了醫(yī)藥供應(yīng)鏈的特殊性,同時研究了現(xiàn)代物流技術(shù)和物流管理理念在醫(yī)藥供應(yīng)鏈中應(yīng)用的必要性。結(jié)合對中國醫(yī)院藥品供應(yīng)鏈的發(fā)展的回顧與歐美日等發(fā)達(dá)國家醫(yī)藥流通行業(yè)與醫(yī)院供應(yīng)鏈管理方法的對比,指出了當(dāng)前我國醫(yī)院藥品供應(yīng)鏈管理中的短板與不足。本課題以X醫(yī)院、上海M區(qū)藥品供應(yīng)鏈改革和D醫(yī)院南院SPD項目為例,研究了早年國內(nèi)醫(yī)院藥品供應(yīng)鏈改革的主要模式,探討了由醫(yī)院本身、政府和醫(yī)藥流通企業(yè)主導(dǎo)的醫(yī)院供應(yīng)鏈改革帶來的得失教訓(xùn),厘清了進(jìn)一步改進(jìn)發(fā)展的目標(biāo)與方向。本課題對F醫(yī)院南院的改進(jìn)型SPD項目案例的研究,研探了經(jīng)過完善的SPD管理模式在大型三級醫(yī)院應(yīng)用效果,證明了其實現(xiàn)了醫(yī)藥分離,提高了醫(yī)院用藥效率,改善了藥品可追溯性,使藥學(xué)專業(yè)人員回歸臨床,降低了全社會的綜合用藥成本,獲得了較好的經(jīng)濟(jì)效益和社會效益,同時指出了進(jìn)一步提高藥品可追溯性和完善法規(guī)監(jiān)管降低運(yùn)營風(fēng)險是未來研究的重點(diǎn)。
[Abstract]:At present, the medicine circulation market of our country enters the strategic transition period, the market growth slows down, the industry competition intensifies, the medicine supply chain service extends to the hospital reduces the operating cost to expand the market occupation, is the medicine circulation enterprise competition new domain. At the same time, with the cancellation of drug addition rate and the strengthening of supervision brought by the implementation of the new GSP, the hospital pharmacy department has changed from the original profit department to the operating burden. Reducing the cost of drug use and improving the efficiency of hospital operation have become the focus of the hospital operators. SPD model is a domestic drug supply chain management model in recent years, which combines the concept of third party logistics management and centralized management ideas. It is an effective way to reduce the cost of drug circulation and improve the level of drug administration in hospital. This paper starts with the basic concept of supply chain management, discusses the particularity of pharmaceutical supply chain, and studies the necessity of applying modern logistics technology and logistics management concept in pharmaceutical supply chain. Based on the review of the development of Chinese hospital drug supply chain and the comparison between the medical circulation industry and hospital supply chain management methods in developed countries such as Europe, America and Japan, this paper points out the shortcomings and shortcomings of the current hospital drug supply chain management in China. Taking X Hospital, Shanghai M District Drug supply chain Reform and SPD Project of D Hospital as examples, this paper studies the main models of drug supply chain reform in domestic hospitals in early years, and probes into the hospital itself. The success and loss of the hospital supply chain reform led by the government and the pharmaceutical circulation enterprises clarify the goal and direction of further improvement and development. In this paper, the case study of the improved SPD project in the South Hospital of F Hospital has been carried out, and the application effect of the improved SPD management model in the large tertiary hospital has been explored. It has proved that it has realized the separation of medicine and improved the efficiency of drug use in the hospital. It has improved the traceability of drugs, brought pharmaceutical professionals back to the clinic, reduced the cost of comprehensive drug use in the whole society, and obtained better economic and social benefits. It is also pointed out that further improvement of drug traceability and improvement of regulation and regulation to reduce operational risk are the key points of future research.
