【摘要】：2010年,北京市在全國率先開展家庭醫(yī)生式服務(wù)的試點工作。2011年3月11日,《北京市社區(qū)衛(wèi)生家庭醫(yī)生式服務(wù)工作方案》出臺,標(biāo)志北京市的基層醫(yī)療機構(gòu)的改革在全國的示范和引領(lǐng)。同時隨著醫(yī)改的不斷深入,時代又賦予了家庭醫(yī)生式服務(wù)全生命周期健康管理、醫(yī)養(yǎng)結(jié)合等新的服務(wù)內(nèi)涵。2016年4月,中央深化改革領(lǐng)導(dǎo)小組會議審議通過了《關(guān)于推進家庭醫(yī)生簽約服務(wù)的指導(dǎo)意見》,簽約服務(wù)將成為2016年及之后一個時期我國醫(yī)藥衛(wèi)生體制改革的基礎(chǔ)性工作。右安門社區(qū)衛(wèi)生服務(wù)中心,2010年成為北京市家庭醫(yī)生式服務(wù)的首批試點單位。5年來,社區(qū)以家庭醫(yī)生為核心,以家庭醫(yī)生服務(wù)團隊為支撐,通過簽約的方式促使家庭醫(yī)生與簽約居民建立長期、穩(wěn)定服務(wù)關(guān)系,從而為簽約居民提供連續(xù)性、協(xié)調(diào)性、綜合性基本醫(yī)療和公共衛(wèi)生服務(wù)的新型社區(qū)衛(wèi)生服務(wù)模式,基本實現(xiàn)大部分居民的健康問題可以在基層醫(yī)療衛(wèi)生機構(gòu)得到解決的階段性目標(biāo)。本研究以北京市豐臺區(qū)右安門社區(qū)衛(wèi)生服務(wù)中心的社區(qū)實踐為個案,以公眾對家庭醫(yī)生認(rèn)知為研究視角,通過對家庭醫(yī)生式服務(wù)的供方、需方角度進行調(diào)查,了解北京市豐臺區(qū)社區(qū)衛(wèi)生服務(wù)中心家庭醫(yī)生式服務(wù)工作運行五年來的發(fā)展現(xiàn)狀,研究和分析目前其運行中存在的不足與問題,主要有高素質(zhì)人力資源不足,服務(wù)能力與服務(wù)功能定位離社區(qū)居民的期望存在差距、區(qū)域協(xié)同服務(wù)機制尚需完善、缺乏建設(shè)高效的信息化平臺、醫(yī)保等配套政策聯(lián)動效應(yīng)不明顯等。并結(jié)合國內(nèi)外家庭醫(yī)生式服務(wù)模式運行狀況,提出進一步改善家庭醫(yī)生式服務(wù)的措施,健全完善醫(yī)養(yǎng)護一體化家庭醫(yī)生式服務(wù)的新模式。同時,為北京市社區(qū)衛(wèi)生服務(wù)機構(gòu)開展家庭醫(yī)生式服務(wù)提供理論參考,為相關(guān)部門制定符合本地區(qū)實際的政策意見提供科學(xué)的理論支持。本研究對進一步推動北京市家庭醫(yī)生式服務(wù)內(nèi)涵的建設(shè)和社區(qū)衛(wèi)生服務(wù)的深化改革具有一定的現(xiàn)實意義。

At the same time, to provide a theoretical reference for the Beijing city community health service institutions to carry out the family doctor service, to provide scientific theoretical support for the relevant departments to formulate the practical policy suggestions. This research is to further promote the Beijing City family doctor service connotation and the construction of community health service reform has practical significance to a certain extent.
